Transfer Out Request Form

    Upon successful submission of this form, a notification will be sent to the International Student Services (ISS) office at Cumberland University documenting the student's intent to "transfer out." Once Cumberland has transferred a student's SEVIS record to another institution, the student will receive an email confirming the transfer, and Cumberland will no longer have access to the record. 

    The student must include a copy of the acceptance letter from the new academic institution below to successfully initiate the transfer out process. The institution must be a SEVP-certified school and the SEVIS school code for the transfer-in school is required. Please ensure accuracy of the SEVIS code for the school where the record should be transferred BEFORE submitting this form!
